LIST OF ATTACHED DOCUMENTS (Annex No. 1 to Government Decision No. 862/2015)

In the case of a request from the Commission for Emergency Situations of the local public administration authority of the second level and special level, the following are submitted:

1) the request of the Commission for Emergency Situations of the local public administration authority of the second/special level to the Commission for Emergency Situations of the Republic of Moldova, submitted through the State Chancellery, containing information on the related circumstances, measures taken with own resources to liquidate the consequences of the emergency situation, request for financial means (expressed exactly);

2) the minutes of the meeting of the Commission for Emergency Situations of the local public administration authority of the second/special level regarding the examination of the situation created, which specify the following:

a) information on measures taken to liquidate the consequences of the emergency situation at the district/Chișinău, Bălți municipality/ATU Gagauzia level;

b) information on the insufficiency of financial means necessary to liquidate the consequences of the emergency situation;

c) information on the insufficiency of financial means necessary for the elaboration of justifying documents (technical expertise report/project documentation and verifications), as the case may be;

d) the decision to address the Commission for Emergency Situations of the Republic of Moldova and request financial means (expressed exactly) and the final beneficiary/final beneficiaries;

3) the extract from the reserve fund of the district/Chișinău, Bălți municipality/ATU Gagauzia (according to the model from Annex No. 2), which demonstrates the insufficiency of financial means necessary to liquidate the consequences of the emergency situation;

4) the request of the Commission for Emergency Situations of the local public administration authority of the first level to the Commission for Emergency Situations of the local public administration authority of the second/special level, which will contain the request for financial means (expressed exactly), material goods, related circumstances and measures taken with own resources to liquidate the consequences of the emergency situation;

5) the minutes of the meeting of the Commission for Emergency Situations of the local public administration authority of the first level regarding the examination of the situation created, which will specify the following:

a) information on measures taken to liquidate the consequences of the emergency situation at the mayor's office level;

b) information on the insufficiency of available financial means to liquidate the consequences of the emergency situation;

c) information on the insufficiency of financial means necessary for the elaboration of justifying documents (technical expertise report/project documentation and verifications), as the case may be;

d) the decision to address the Commission for Emergency Situations of the local public administration authority of the second/special level and request financial means (expressed exactly), material goods and the final beneficiary/final beneficiaries;

6) the expense estimates for carrying out construction/ reconstruction/ repair/ demolition works, elaborated on the basis of the Technical Expertise Report/project documentation, as the case may be, and verified in the manner established by legislation;

7) copy of the financial document/documents justifying the costs for elaborating the project documentation, as the case may be;

8) the extract from the mayor's office reserve fund (according to the model from Annex No. 2 to Government Decision No. 862/2015), which demonstrates the insufficiency of available financial means to liquidate the consequences of the emergency situation;

9) the application of the individual to the Commission for Emergency Situations of the local public administration authority of the first level regarding the request for financial aid or material goods;

10) copies of identity documents, in the case of aid requests by individuals, including their consent for processing personal data;

11) certificates attesting ownership of the dwelling, as well as the cadastral value according to data obtained from the Territorial Cadastral Office (as the case may be);

12) the act of establishing the damage, drawn up by the Commission for Emergency Situations of the local public administration authority, with the mandatory participation of a representative of the General Inspectorate for Emergency Situations of the Ministry of Internal Affairs and an expert in the field in which the emergency situation occurred;

13) the acts drawn up by the mayor's office regarding the availability of dwellings that can be purchased on the territory of the locality and their cost, as the case may be;

14) the declaration on own responsibility regarding the truthfulness of the information from the set of justifying materials and the substantiated calculations presented to the chairman of the Commission for Emergency Situations of the local public administration authority of the first, second and special level (according to the model from Annex No. 3 to Government Decision No. 862/2015);

15) justifying/confirming materials, accompanied by photographs, maps, schemes, certificates that characterize the addressed problem, as the case may be.
